The error is constant throughout that list:

Error: MySQL server has gone away

Typically (but not 'always') that's something you need to ask your host about. Provide them with the log or the last few entries, and see what they say. ;)

Occasionally a resource heavy modification can cause that, but that's not common and you normally get other issues if that happens too. Your host should be your first point of call to ask about this anyway, as it is a server side issue.

I do see entries related to a shoutbox and attack system, but as above, see what your host has to say first.
