There's documentation available in the package you downloaded which should guide you through that process, and probably more.

No, you upload the contents of the upload folder in the .zip, NOT the upload folder itself.
Try not to confuse it with the uploads directory present already.
Having the 'contents' of the upload folder in the .zip ready you would just upload these to where your board is installed.
If your board is at www.example.com , typically just inside public_html
If your board is at www.example.com/forum , it will usually be public_html > forum

Then upload the contents of the upload folder (in the .zip) to your boards directory via FTP. Do not upload the 'upload' folder, just the two folders (admin and public) inside it.

Correction, v1.1.2 is the one you have to use for IP.Board v3.0, v1.1.3 is for 3.1 only.

1.0.2 was the last version of IP.Shoutbox for the IP.Board 2.3.x series.
The versions for 3.0.x are 1.1.0 upwards (excluding 1.1.3) although without testing an install on 3.0.3 I cannot be 100% sure which version is suitable. If I had to take a wild guess, I would say 1.1.1 will work with 3.0.3. Possibly 1.1.2 will but its not tested as 3.0.3 was old when 1.1.2 appeared.
Obviously the sensible course of action is to upgrade board to at least 3.0.5 if not 3.1.2
